Track and Evaluate Your Therapy Trial

A therapy trial may last weeks or months. During that time, symptoms may improve, worsen, or change; side effects or other events may occur; and some changes may have causes unrelated to the therapy. It can be difficult to remember later exactly what happened and when.

Keeping a written record can help. Record:

  • Changes in your symptoms and functioning,
  • Progress toward your Primary Target,
  • Your experience with the therapy,
  • Possible side effects, and
  • Other events or changes that seem unusual or potentially important.

Record what you observe and when it happens; you and your physician can consider later whether an observation may be related to the therapy or to something else.

The Tracking Sheet (TS) shown in the Roadmap above is used during your therapy trial. The Completion Questionnaire (CQ) is used at the end to help you evaluate what happened and whether the therapy provided the benefit you were seeking.

If your physician provides forms for tracking and evaluating your therapy, use those forms. If not, you may download and use the one below. You may print this form for handwritten observations or use the Word version to type your observations.
